We
offer the following services to people who experience
mental ill health:
Welcome to Mobile
Community Teams
The
WALSH Trust Mobile Community Teams currently supports
around 160 people who experience mental ill health.
Each client contributes to a needs assessment and
support plan. The focus is very much upon their
strengths, and their recovery; ‘what will
be happening for you when you no longer need our
services?’
Community
Support Workers typically focus upon the practical
day to day supports that
help people who experience mental ill health,
to navigate or overcome barriers to attaining and
maintaining the basic elements of a ‘regular’ life.
This is achieved through assessing and building
upon the strengths each person possesses, while
at the same time addressing the challenges that
may inhibit a person’s recovery.
The
elements might include decent housing, satisfying
employment,
meaningful social roles and relationships.
The barriers can include poverty, stigma and discrimination,
disconnection with family and friends and social/community
networks. Community support can also facilitate
access to peer support services which focus
upon
supporting people to develop their social networks.
This service is for adults, aged 17 and over:
- who are resident of Waitakere City
- who have been referred by Waitemata DHB mental health services *
* Where capacity permits, we are able to receive applications from people diagnosed by a Doctor (e.g. a general practitioner) as currently experiencing mental ill-health.
